How the program works:

1. Complete the registration form below.

2. Your friend will receive an email or a phone call from us to notify them of this opportunity.  (You can tell us if you would like to remain anonymous and we will not release your name.)  

3. 30 days after your friend makes a purchase of at least one hearing aid, you will receive a $50 Visa Gift Card and they will receive a $50 discount.
